Earthquakes, tsunamis, hurricanes: that's what most of us picture. But some of the deadliest disasters in history began with something people built. This episode counts down ten of them: the deadliest man-made disasters where the public record shows a document, a permit, an inspection or a decision that let the dam, the ship or the plant keep running.

Chernobyl isn't on the list: by 2005, the U.N. agencies that reviewed it reported fewer than 50 deaths directly attributed to its radiation, while the projections run from 4,000 to far higher. The ranking uses the latest official count wherever one exists (for the Doña Paz, the Philippine Supreme Court's figures); the missing and the higher estimates are given beside each number. Attacks and acts of war are excluded.

From the Toya Maru, 1954, to the Banqiao Dam, 1975: the Titanic, Halifax, Le Joola, Vajont, Johnstown, the Doña Paz, Derna and Bhopal, with how the public reacted at the time. Every claim is sourced to a public document and each conclusion is attributed to the body that reached it. In Derna, the retrial had reached no verdict as of September 2026. No images of victims are shown.
